Job Description\n

American Traveler is seeking an experienced RN Case Manager with a BSN and Epic proficiency for an acute care hospital setting in NC.

\n \n Responsibilities\n

• Work in a large hospital with trauma level III designation and multiple centers of excellence
• 8-hour day shifts from 8:00 am to 5:00 pm, Monday through Friday
• Caseload averages 14-16 patients per day, dependent on assigned unit
• Possible floating to any unit within the hospital
• Patient population includes adult medical/surgical, cardiac, orthopedic, and occasionally pediatric cases
• Use of Epic EMR is required for all charting and documentation
• No weekend or on-call requirements
• Holiday coverage required unless approved off in advance
• Role focuses on assessment, care management, and discharge planning for diverse patient populations
• Collaboration with supportive interdisciplinary team including Director, Managers, CMO, UM team, social workers, and peer case managers
• Orientation period ranges from 2-5 days depending on prior experience
• Scrub color is navy blue
• Supportive and teamwork-oriented department culture
• Free parking is available on site
• Manager approval required for requested time off
• First-time travelers are not eligible for this position

\n \n Requirements\n

• Active NC or compact RN license
• Bachelor of Science in Nursing (BSN) required
• Current BLS certification required
• Minimum of 3-5 years of case management experience required
• Prior RN Case Management travel experience required
• Epic EMR experience required
• Care coordination and discharge planning skills required
• Ability to manage complex placements for SNF, IPR, LTAC, hospice, home health, DME, behavioral health, and homelessness-related needs
• Case Management Certification preferred
